I believe that if you start a private match with 3 of your best friends and ask them to get into a line (one behind the other)and find a spartan laser and you should get the 2-for-1 with the spartan laser.

The Extermination medal, you have to kill the entire five-man, opposing team within four seconds of each other.
Perfection is in a multiplayer match where you kill 15 players without dying a single time. Hence, perfection.
