You may have done something wrong , try the level again , and look at the cheat once more. Maybe you did get it but due to a temp glitch it didn't appear in the achievements.

Some of the levels need to be done on a certain difficulty so either that or you want to try the level again. Maybe you are not suppose to get one

Go through it on normal or legendary. Make sure to start from the beginning though.
