Yes, you can get skulls on all difficulties except normal.

Yes you can get skulls on normal heroic and legendary.you can not get them on easy.Also if you start a mission at a rally point the skull won't be there.you have to play the whole level out.

Yes, you can gt skulls on any difficulty xcept easy. The skulls won't appear if you start from a rally point so you have to start from the beginning of th mission and they are pretty easy to find and get.
