I have 1, 2, 3, and 5 I can't find the location in any walkthrough

I don't know which one is actually 4...my guide says 4 is in hallowed moors cemetery which is between Big Town and Germantown...in case thats the wrong one here is the list of all of them...
1-grisly diner
2-abandoned shack east of Rockbreaker's Last Gas
3-wastelander pylon due north of WAPL-58 power station
4- hallowed moors cemetary
5-military tent north of anchorage memorial, the correct tent has a super mutant captive in it
