Soz, but yes they do have to die if you want the true ending. The true ending can be completed if you unlock Overtime Mode by completing all cases, speaking to Isabella at around 10am on the last day at Carlito's Hideout and getting to the Helipad by noon.
Then in Overtime mode, you have to collect 10 queens, collect 10 items and get a generator. Yoiu will then have to fight through a sea of zombies and finally defeat the final Boss, Brock. You will then unlock infinite mode.

Yea they do I never found out what happened to brad though last I saw him he went after carlito and got traped with a crap load of zombies
