It depends on which mode you are playing. If your playing SURVIVOR mode, the game is over completley and you must start a NEW file.
If your playing on normal mode, you will start as a completly new character or person. The Twist is, everything you did with your recently deceased will have affected the in game world.
For example, you die and have a backpack full of flares. When you wake up in the safe house as someone new, you will need to find your old character. That character is most likely a Zombi now. Although He/She should be using your backpack with the flares. So if you kill the Zombie YOU get your stuff back. This goes for other things in the game as well.
