You need to make at least 1 custom belt and go to exhibition. Go to Title Match and pick Custom title match. Pick a match ( particularly singles match if the title is single ) then pick your superstar and fight. If you win, you get the belt and the superstar even wears it.

Are you talking about in the Season Mode version of Smackdown vs Raw 2007? If that is indeed the part of the game that you are talking about, then there is only small thing that you need to do so that your character can wear his or her championship title belt. Play Season Mode and try really hard so that you win all of your matches. When you win enough matches you should get a message on your phone from Jonathan Coachman that tells you that you have been granted a title shot at your next pay-per-view match. If you are talking about in Exhibition mode, or in regular battles then you will need to do a title match and then select the belt that you want to win. Select the belt and then select the match type that you want, pick your person, and then win the match. During your matches to defend the belt, he will be wearing his belt.
