You don't have to kill them all. You have to do a violent, and a gruesome kill with the hammer. Then kill the person with the bat, and use it to do a hasty, violent, and gruesome kill.
Hasty = go behind an enemy, and quickly do an execution.
Violent = go behind the enemy, and lock onto him long enough for the target to turn yellow, then do the execution.
And
Gruesome = Go behind the enemy, and lock onto him long enough for the target to turn red, then do the execution.
After all this the exit should open.
