Here is how to beat it.
When you get in the room with the ice cubes, swallow the fire torches, then melt the ice
Cubes, and you will be able to turn into baby Mario. Once you get to the part where the
Key is you should be able to shoot a deflection shot at the ? Cloud and release the key,
Then shoot another deflection shot to open up the door, so that key will fall through. You can then opened the locked door and you will be able to get to the area with the !
Button, jump on that and you will turn off the lava flow and then you can easily get
Number ball 4 and pretty much to the end of the level.
