missing the last piece of the map!

missing the last piece of the map!

i have completed all stories with all 4 people, yet i am missing a piece of the map????? this makes no sense to me, and i cant figure out what im missing or doing wrong, does anyone have any idea what i should do now?

Lower right hand corner:
Flower Patch (above the area with a crafting rock). It will be there from the
start of the game as a small square of parchment.

Upper left hand corner:
Sparkly cave w/ skeleton. When you first enter the sparkly cave, you should
see it lying on the ground as a very small piece of parchment.

Lower left hand corner:
By the pirate doors. This one is located at the stone doors with the skull,
and at the left hand side door bar (to your left) as a very small piece of
parchment.

Upper centre piece
Suzy McDress's map piece. You'll need to finish all her quests in order to
obtain this particular map piece.

Upper right hand corner
Dr. Feelgood's map piece. You'll need to finish all her quests in order to
obtain this particular map piece.
