How do I get final star (and 5th wallpaper)?

Okay I beat the entire game, collected and spent all 240 gold coins, beat the necessary bosses with mini-mario, took every alternate exit and opened up and connected every path but STILL the 5th wallpaper is grayed out. What did I miss?!? (I can play in challenge mode too). Help!!

The 5th wallpaper is unlocked once you have collect all star coins and found all hidden secret exits as well as all the stages. There is quite a few hidden exits and just about every ghost house has one too.
