Simply set up the Level Keyes on Heroic with the Bandana Skull active and then when the level starts make your way to the end of the passageway where you will need to jump down from the open door and destroyed deck. Now turn around and head back to the hall and start chucking grenade after grenade down the hall to the Squid spawn point. You will unlock the Achievement in around 30 seconds!
