You buy it at the junk shop.
You could either buy it at the junk shop or Martha will upgrade it if you come into the clinic with yarn and she has 2 or 3 hearts.
You can either buy one, and then there is a second larger option, where if you befriend Martha, she will offer to make it bigger if you bring her some yarn.
