Question for The Legend of Zelda: A Link to the Past

I just got the moon pearl, the medallion and the master sword.

What do I do next?. When do I get the hammer and the hookshot?.

It seems that there are only certain places I can go in the Dark World, is there some kind of portal I'm supposed to go through.

1 above the biggest city, the one in the west

2 going to the desert, if you go south through the opening in the mountain, you will go west. You cannot use this one til you get the hammer to knock down the poles.

3 on the island in the middle of the ocean, that has the entrance to the fairy lady, you pick up that rock, and there's a portal under there.

4 up in the mountains in the north in a circle of rocks or something. There's a few others, but I can't think of them off the top of my head...

To get to other places in the Dark World you need to get more items.

I believe you need to go to the big skull on the map. This should be at hyrule castle. This will only be true if you have not defeated Agnaham for the first time.

The hammer is in level 1 in the dark world, and the hookshot is in level 2 in the dark world
