I think it is so you can put your mii head on? Try to click the head and see if it changes your mii to the way it looks in other games. A plus to having your mii head is when you get stung by the bees you don't have to take the meds.
The mii sign is to hold a mii stile. Go to gracie grace in the city and tell her you want a makover. Then choose a mii from the mii channel. If you don't want to wear it anymore, go to designs and click on the miis head icon. You can take it on or off, and you can get a different one every other day. You can't get something else in the same day.
