I have made all of them and never got a surprise..unless I havent received one yet but I believe I have them all..

If by "surprise" you mean - "keep making accessories and see what happens" when you click on title or whatever. You have to make all of the accessories and get a perfect 5 sparkley flowers. And then you will get the very last title of the game.
