The 'Transmissions' and some of the contagious 'Symptoms' you select in Plague Inc: Evolved are what help spread your disease from the original location it starts. How quickly your disease actually spreads in the game after that is reliant on luck and pure chance.

After you have selected the country you want your disease to start you need to start developing some 'Symptoms'. Consider Sweating, Rash, and Skin Lesions as these are capable of increasing infectivity at a low price. After 'Symptoms' you move into 'Transmissions', if you have started in China as your starting country you should consider Rodent 1 which is particularly effective in urban areas. If you selected Saudi Arabia you should opt for air and water 'Transmissions' first.

When making your decision you should immediately avoid countries that are small islands such as New Zealand, Greenland, and the Philippines as it will take longer for your plague to spread as they have a low amount of incoming and outgoing ships which is the only way the infection can spread. The best countries to quickly spread your disease are China and Saudi Arabia.
