Turning the diffuculty bar all the way to hard doesn't make more monsters appear. It just makes the ones that are already there really hard to kill. I turned it up on hard and tried killing the count of bravil once by using a sneak attack but it barely took his health bar down at all. I'm not sure if you take more damage or not but it does make the enemies more aggressive also.

Heh, I forgot to answer this part in my last answer, about the black soul gem. The place is called dark fissure cave. It is located southeast of Cheydinhal. It's to the right of the "R" in the "River" part of that rivers name southeast of Cheydinhal.

No, there will still be the same monsters they will just be a lot harder to kill. On the other note, I don't know how to make black soul gems either.
