Well, let me give you a checklist even before you start the mission:

- Ship Main Cannon Upgrade (Recieved from Garrus)
- Ship Shielding Upgrade (Recieved from Tali)
- Ship Plating Upgrade (Recieved from Jacob)
[For Every one of these you don't have, you'll lose a teammate even before getting to the base. Have all of them before beginning]

- ALL loyalty missions complete.
[While it may be possible to keep everyone alive even without them Loyal, it's much easier just to have them all loyal. So do it.]

- Obviously, have your entire crew, here's a checklist just in case:
1. Miranda
2. Jacob
3. Garrus
4. Mordin
5. Grunt
6. Tali
7. Thane
8. Samara/Morinth
9. Jack
10. Legion
11. Zaieed

Now, this order is very important when going in:

1st Choice:
Team Leader - Jacob, Garrus, or Miranda
Tech Specialist - Legion or Tali
[Note: Anyone you don't have Loyal that you have as a leader or specialist will probably die.]

2nd Choice:
Team Leader - Jacob, Garrus, or Miranda
Biotic Specialist - Samara/Morinth or Jack
Escourt - Any Loyal Teammate (Preferably Mordin)
[Note: Technically you can let your crew die on the way back and you can still get the achievement, but you want to send Mordin back, as he has a high chance of dying in the final stage of the level.]

Final Choice:
Team - Any two Loyal Teammates (Preferably Jack and Tali)
[Note: While you go to the final battle, your teammates are holding the Collectors off. Each team member has a score to "Hold the Line", and if their total score isn't high enough, someone dies. Mordin, Jack, and Tali, according to Bioware, have the lowest of these scores. So if you send Mordin back and take the other two and leave you big guys to hold the line and they should all make it just fine.]

And that's it! If you follow this, you should survive the final mission with your whole team in tact.

When i completed the game i didn't have legion's loyalty, but i was able to keep him alive. i just made sure not to put him in any role and i did his research project before the final mission.
