I unlocked it on act 2 wer you enter the factory looking for the cart control room, anyway just after you have entered the factory and you find a stranded and he leads you to the bit with the wooden floor which he falls through and gets killed by wretches and you have to find the right path through, just fall down on purpose and get them to chase you until the you think you have a few wretches following you then pin a grenade on 1 of them(melee atack[B button] with grenade equipped if you didnt know),if you die it doesnt matter, you will just end up at the beggining of this part,u might got get 3 or more each time but you most likely will, repeat until it says achievement unlocked, hope this helps :D

I got mine in act 3 when you can fall through the floor, you get to that part and fall through the floor wait until 4 or 5 exploding wretchs are following you and tag on with a frag gernade, to the 10 times and you have you achievement
