1. The sub level is locked before the quest 'Who Dares Wins'. There is no key.
2. The formula is located in the R&D section in the safe on the wall of the Nuka-Cola plant and is located in the first area to the left, after entering the plant. You must get the key for the safe from Milo, Shipping Foreman.

Well I have no idea about the white house code but the formula is in the safe and the overseer mister handy guy has the code he should be somewhere by the way the code is NCC-1864 which coincidentally the service number of the USS Reliant from Star Trek II Wrath of Khan
