Get married and purchase a home.
Then; when the time is right she will hint for sex.
Use the 'thumbs up' expression; and lead her to a bed.
Then put the bed to good use.
If you have a condom; you will be asked for unprotected or protected sex.
Obviously; if you wan't a baby pick unprotected.
However; he aware you CAN get Sexually Transmitted Diseases (STD's) in the game; so do this at your own risk.

Go to any town you can own a house in. Find a cute girl and hit on her until there's a red heart above her head. Then give her a ring depending on her social status. She will then follow you. Lead her to any house you'd like her to live in that you own, talk to the sign, and set it as your marital home. You'll marry her, and then she'll offer to have sex with you. Give her the thumbs up and then go to your bed and hit A. Choose unprotected sex and get it OOOOOOOOON!
If you did it right, there will be a message and a little cutscene that tells you that you've got a kid. Be careful, unprotected sex can lead to STD's.
