In order to get the moon map you need to buy the rezzurection map pack, it comes with the old zombie maps like der reise etc.
It also comes with a moon theme for Xbox 360 and the COD:BOZ (call of duty black ops zombies) cd dowload.

It's a DLC map that you have to purchase in the Xbox Marketplace. Last I checked it's $15 USD.
