When you have the most players named to your league's All-Star team during any season the 'Big Head' card will become unlocked. You will need to play through at least 10 games in their entirety (no simulations) by the All-Star Break to achieve this.
Enter the following case sensitive codes (without the quotes) at the 'Codes' menu which can be found in the 'Trading Card Album' to get the corresponding Classic jersey.
Unlocks ALL of the American League East Classic Jersey Trading Cards:

Enter the following codes at the 'Title' screen to unlock the corresponding feature in the game.

If you enter the password 'Derek Jeter' at the 'Password' entry screen everything in the game will become unlocked.
