You can't have a child, but if you want to have sex with her, just keep talking to her over and over again until she gets a green glow around her.

You can't have kids in this game. But, if you want to have sex with your wife, then just get naked and keep on hitting 'A' while looking at her. Eventually the outline around her will turn green, when it does, hit 'A' one last time. A pop up should come asking if you want to have sex with your wife, hit 'A' one last time.
