Every time I get his health down he kills me is there a weak spot

Keep shooting him in the face. If he has a 'weak spot' then it would be his face. He loses more health should you shoot him there.

When he has his tentacle move through the ground to get at you, shoot them whilst they are underground to stop them, if that's what keeps killing. I could help you more if you could be more specific.
