To brew a "smart milkshake" you need to go to a workbench to make one, you need a cooking skill of 4 (I think it's 4...) and some "Smart Milk", the Smart Milk can be obtained in the Kine Dairy in Deadwood (It is only unlocked after you complete every mission in paradise place) And thats how you make it.

You need to find first a workbench, then of course smart milk, you can buy a smart milk in the Kine dairy society. Go to the cashier then select buy and find the smart milk then buy as much needed to make smart milk shake potion. And as Pro-Gamer said you must have a cooking skill of 4. You can do it by cheat or watching a cooking show in a television.
