Complete the Story Mode on Normal difficulty.
Unlock Stars and Stripes Skin Pack:
Complete the Story Mode on Hard difficulty.
Unlock Laser Pistol:
Complete the Story Mode on Twisted difficulty.
Unlock Warthog:
Complete the Story Mode on Twisted difficulty with ALL gold medals.
