There is no direct card for the Perfectly Ultimate Great Moth available in Duelists of the Roses, only the fusion of one of a select few insect monsters (Examples: Petite moth, Larvae Moth...) with the Cocoon of Evolution. For how amazingly awesome the Card is, it must have drawbacks of course. It is visible in the tutorial to see this monster summon able in normal terms, though this is the only time you'll ever see that circumstance.
If you want the Cards to make it, they are all obtainable from Weevil Underwood, one of the first White Rose Duelists.
