You have to get excellent on the "Battle Finale" chapter.
To do this, you have to get stars by three of the four categories... just like every other chapter.

To get the excellent, you'll have to have over 70% accuracy, good time, and a good combo.

Mr. Kelly ALWAYS hit me, and does at least 60% damage every time he hits you... and that's if you have all the health increases.

If you’re very, very good, and he doesn’t hit you, and you have good accuracy, you shouldn’t have to worry about a combo to get the excellent… but I’ve played that level maybe 100 times, and he’s always hit me.

(He’s very, very fast) My advice is to beat the game once so you have the scorpion gun, hit him three times with critical hits in the duel (to spawn the cut-scene) and then kill at least two citizens and him in the same combo.

That's what it took for me to get it. Don't kill too many citizens though, or the game will make you re-play the level.

It took me a good 30 minutes to get the excellent on this level... but you'll get it, as long as you don't get frustrated and give up. :o) Oh... and if you've already done some bounty hunter levels, golden gun, invincibility, bite the bullet and infinite dead eye can help... although bite the bullet may make him die too quickly for the combo to get high enough and trigger the excellent.
