I got one from Kakashi at Ichiraku. Go to the Merchant near the middle of the Leaf Village Center and buy everything he has. (Trust me, it's worth it.) Afterwards, go looking around for Kakashi. He could be in a bunch of different places. When you talk to him, make sure you give him the item you're asked about. He'll accept it. That's when you head to Ichiraku. Talk to him, and he'll leave. Find him again, and give him another item. Return to Ichiraku. This time, he should go inside with you. He'll claim to have to go to a convention and leave. Before he goes, he should give you the Scarred Headband.
