When you unlock it you automatically put it on for the rest of the game.

You unlock the apache shirt but it doesn't change your appearance in any way

Apaches don't actually wear shirts. By Apache "Shirt", it's actually referring to tattoos, which can be seen on Cole when he's wearing certain outfits.
