The game doesn't show whether or not Hawk dies. I don't think he does, since he's carried away concious by the medics.

If you are talking about the part where he gets shot by the sniper and you have to get the medic but the medic gets killed by some guy in a building {my friend thinks it's the sniper but you never hear him shoot the rifle when the medic is killed} then yes he does survive.
