You need to complete the main storyline missions, as well as all the side-missions. That includes:-
- buying all the properties/safehouses (shack and condo near the stadium, the one near Prawn Island, the one near the golf course, the one near Pole Position club, etc)
- completing the missions for the businesses, such as Cherry Poppers (drugs), printing press (counterfeiting) and car showroom (completing car lists by driving the cars into the garage at back)
- completing the RC missions (like the army helicopter toy at the side entrance to the airport)
- complete the Ambulance/Police/Pizza/Taxi/Fire Truck missions (use the hospital near Washington Beach for the Ambulance mission, and use the 180 degree spin often; unlock the apache first, then steal it from the army base for the Police missions, as it fires missiles; do the Taxi missions near North Point Mall, as there are many shortcuts; do the Fire Truck missions near the stadium)
- collect all 100 Hidden Packages (green statues); I think collecting all unlocks the apache/Hunter, which appears inside the Army Base near the airport (wear the Police Uniform to get in without being shot at - located inside the locker room of the police station, south of The Malibu)
- completing all the Rampages
- complete all the Unique Jumps (some require you to use a motorcycle or sports car)
- compete (and complete) in the events inside the stadium
- race on the dirt bike track near your Hyman Condo, both with Sanchez and with the off-road vehicle
- rob stores, like the hardware stores, jewelry stores, pharmacies, stores in North Point Mall, etc (basically any store with a cashier who doesn't carry a gun)

There are a few mini-games, like Cone Crazy (learning to drive on the rooftop of a parking lot near Washington Mall), PCJ Playground (bike between The Malibu and police station, used to do Unique Jumps over the buildings), shooting seagulls with a laser scope, and the Sparrow (lightweight helicopter) missions (can't remember where/when they appear, but I remember there was one on grass, in the backyard of some apartments, possibly close to North Point Mall/golf course).
I'm not sure if these mini-games contribute to the 100% completion or not.
