First you have to have 50 Bone dust, they are dropped from undeads in Swordhavenundead or any undead map. Second you have to go to citadel, go all the way Right till you get to a kinda Portal, then you in that portal. Third, once you are in Tercessuinotlim, kill the Dark Makai to get Essences of nulgath.
NB: each 100 essences of nulgath turn in 1 totem. Essences are NOT 100% drop.
