Hey! Well firstly you have to get the dragon tattoo FIRST and then you can get the grannie panties!!
Now, talk to that hippie guy with the long black hair and buy him a beer. Ask him for the grannie panties, then go through a few steps but make sure you don't lose him. Then ask him AGAIN about underwear and hopefully his JOKO will come. Dont say anything, just pull out the camera found in 'use item' and JOKO has the dragon TATTOO so just take the damn PHOTO!!
