It is in Turnback cave, I don't know the exact way to get to the Distortion world. But I know you need to beat the Elite 4 and Cynthia. Spring path is at the right in route south of Veilstone city, you can enter Turnback cave there, and defog it. Go into the caves, experiment! Voila!

After you have defeated Team Galactic in Veilstone City, head up Mt. Coronet using the Hearthome City Entrance. At the top of Mt. Coronet is Spear Pillar. After you defeat Team Galactic on Spear Pillar, you will go into the Distortion World. At the end of the Distortion World is Giratina. You must first defeat Cyrus before you can battle Giratina. After you battle Giratina, you will leave the Distortion World. After that, you can go into the Distortion World by using the entrance deep inside Turnback Cave.

There are two ways to open the portal to the Distortion World:
The 1st way is part of the story. You have to infiltrate Team Galactic HQ, then to up Mt. Coronet and find your way to the top, Spear Pillar. After a few battles, the Red Chain will be activated and the portal to the Distortion World will be opened.
The 2nd way is to go to Turnback Cave and go in random cave doors until one cave door is filled with a blue light. Go in it, if you have caught Giratina already, then the portal to the Distortion World will be in the centre, and if you haven't, then you must defeat or catch Giratina, and the portal will be under it.
