1) Absolutely nothing besides Mew's icon appear when you select Continue on the start menu.
2) After clearing Pitfall Valley, and if Blue Sky Meadow Friend Area is possessed, go to talk to Medicham and Ekans in front of Whiscash Pond. Do a mission, talk to them again. Do another mission, only Ekans will be left there, talk to Ekans, then look for Medicham's rescue request at Pelipper Contact House notice board.
3) How? I can't answer how. But there is a hint: use the save point and if you can't recruit him, turn off the game and continue from the save point. Note that you CANNOT recruit him if you haven't beaten Rayquaza.
