First, you need to beat the game and complete the sinnoh dex (like about 150 Pokemon you have to see) and go to pal park.

You need Ruby, Sapphire, or Emerald. If you don't have any of those games, you're out of luck.
If you do have any of them, you need to catch the three Regi's in them.
In order to migrate from GBA Pokemon games to Diamond/Pearl, you need the National Dex. Got it? Good.
You can only migrate Pokemon via Pal Park, which is at the end of Route 221.
Now, save your game, turn off the DS and insert Ru/Sa/Em. Then, play the game [Diamond/Pearl] until you're at the title screen. Along the options should be "Migrate from XXX". Select that. From here, you have to select 6 Pokemon from the PC from the inserted GBA Pokemon game. Select the three Regis and three other Pokemon, and then continue your game. Go into Pal Park, and find the three Regi's in the mountain area. Then, you need to find the three other Pokemon :]
