When you get the national dex, you go to Route 221(South of Sandgem town), you will find Pal Park. After you save, you put in R/S/Fr/lg/E and transfer 6 pokemon. You catch them at Pal Park, they will be the exact Same. Be warned, don't transfer in Pokemon you don't want to lose, because it is One way trip.
You have to have the National Dex and then you should have a option called Migrate when you start up your game. This allows you to move Pokemon from GBA games to your Diamond.
Once you've obtained th National Dex, go to Pal Park at the end of Route 221. Insert a GBA Pokemon game in the DS socket, and save and restart your game. At the main menu, there should be an option named "Migrate from XXX". Select that option and choose 6 Pokemon in your PC in that GBA Pokemon game to be migrated into Diamond. Now, start the game and talk to the guy at Pal Park. He'll give you 6 Park Balls [that have a 100% catch rate] and then you go out and catch the 6 Pokemon according to their terrain.
Well first of all, you must have beaten the elite 4. After that prof. Oak will invite you to Pal Park on route 221. And that is where your able to transfer Pokemon between games.
