Where do I get the pokedex app that shows the different formes of a pokemon(plant burmy, sand burmy, and trash burmy)?

You get from one of rowans aids.I THINK he is near the city with the contest place.

Near the contest hall but by the bicylce place, but to look at them you need to see them in a battle.
