Actually, the Name Rater is the Name Changer. He will rate your Pokemon you choose's name, then he will allow you to change it. The Nickname rater/changer can be found in Eterna City, in the big building right next to the Town Market. He is the elderly man, on the 1st floor.

They are one in the same. He is in the large building next to the Market thing on the first floor.

It's the same thing. The name rater rates your pokemons name then asks if you want to chage it. But you cannot change the name of a Pokemon that you have received through a trade (that belonged to someone else).
