Question for Pokemon Diamond

After you beat the 4th gym what do you do and what town do I have to go to?

You have to go back to Hearthome City and fight the Gym Leader that wasn't there before.

Go back to Pastoria City, if you're not there, already. In the northeastern corner, there should be a Galactic grunt facing the building to the Safari Zone. Talk to him, and then start chasing him. You'll eventually battle your rival, and after you beat him, continue chasing the grunt. He'll eventually stop and battle you. So once you beat him, go up a bit and talk to Cynthia. She'll give you the Secret Potion. With this, go to Route 210 and give the Secret Potion to the Psyduck. Cynthia will come up to you and give you the Old Charm, asking that you give that to her grandmother in Celestic Town. Note that the route to get to Celestic Town is a bit difficult without HM05 Defog. To get this, go back to Pastoria, and go in the Safari Zone. Right when you enter, talk to one of the guys to your RIGHT. He'll give you HM05 Defog.

Once you reach Celestic Town, battle the grunt in the middle, and the old woman will come up and take the Old Charm from you. Go in the cave, and examine the drawing on the back of the wall. She'll come up to you again, and give you HM03 Surf. With this, you can now obtain the fifth badge, back in Hearthome City.
