Ok first of all, you need a wood stick(usally found by tress in jungle looking place) and then a rock(found where you washed up in the begging of the game), then sharpen the rock in the "Making tools" section with the x menu and make the rock hard then select the spear then select the wood stick and the sharpened rock and hit x.
