Go into the Twisted room and exit to go into the attic. Talk to the clock then go inside it and stay on the circle in the middle until the gear above the door have a goblin-like face on it.

Enter the clock in the twisted world, switch to Miles and you will see a small gear on the floor. Touch on it and Miles will place it in the centre of the middle gears, opening the way to the school.
