I've Finished the normal campaign mode and I got three abilities for all the CO's, but there are still two blank spots under those.
What are they, What do they do, and How do I get them?

You get them by finishing hard campaign. As for what they are, they are mistwalker and soul of hachi. Don't know what mistwalker does but soul of hachi lets you deploy from cities when using you're super/tag.

You get them by beating Hard Campaign. Those two skills, Mist Walker and Soul of Hachi. I think Mist Walker lets you go normally in Mist/Fog. Soul of Hachi will allow you to deploy from any city that you own if you're using Super or Tag.
