Just play under multi player 50 times and it will tell you that you have it.

vs mode> time team OR stock team (time or lives).... or once inside the character selection in the upper left part it says "FREE-FOR-ALL" clicking there will switch it to TEAM BATTLE

I played multiplayer 101 times, still no item choose. I'm playing smash 64 on wii, if that makes a difference
