There is a whole task that you must complete in order for your Sims to have a baby. This task should appear when you reach Level 8. However, if you were asking how you add a sim as part of the task; after building the Children's store buy a cot from the store. It should be in your inventory, place it in the home you wish to keep the baby and there should be a bubble that appears on top of the cot, click on that and that will help you with the creation of the sim baby.
