There are two possible problems that I can think of. If they will not change it because a speech bubble with blue footprints shows up, then it means they can't get to the crib and you need to move the crib away from a wall or other Objects in the room.
If they seem to change the diaper but the baby's toilet Bar doesn't improve, that is normal. They are changing the diaper the baby just hasn't gone to the bathroom in it yet. You have to wait until the toilet bar is way way down to the very bottom, then the baby will use it's diaper. The toilet bar will turn green again and the hygiene bar will go way down. Now is the time to change the baby's diaper.
