The dungeons are much harder and different. Your enemies are harder to beat. This game is for the real Zelda Masters! Even I can't beat the game!

The puzzles of the temples have been changed. Location of rare items (gold spiders, picies of heart, ect.) but some are in the same places. That is about it. I found it just a little hard. The real change happens in the third level as young Link.
